1. What this policy covers

This policy explains what information mindfullifechicago.com collects from people who visit it, how that information is used, and who it is shared with.

It covers the website only. If you become a client, the information created during your care is handled separately under our Notice of Privacy Practices, which you receive in your client portal during onboarding.

2. What you tell us

When you get in touch through the website, you tell us your name and how to reach you, your insurance carrier if you have one, which clinician you’d like to work with if you have a preference, when you’re available, how you found us, and a short description of what you’re hoping to work on. You can also give your pronouns, and — optionally — your racial, ethnic or cultural background, which we ask about because some people would rather work with a clinician who shares part of their background.

The form also asks you to confirm that you’re in Illinois, which our licenses require, and that you’re willing to meet by video.

A sentence or two about what you’re looking for is all we need at this stage. There’s no need to set out symptoms, diagnoses, your history or your medications on a web form, and we’d rather you didn’t.

If you are in crisis, please don’t use the form. Call or text 988 (Suicide & Crisis Lifeline), or call 911.

We use what you send only to reply to you, arrange a consultation, match you with a clinician, and answer questions about scheduling, insurance or fees.

If you’re applying for a role with us, our Join Our Team page has its own separate form. What you send there is used only to consider your application.

How it’s handled. Your message reaches us as email. It isn’t added to a client record unless and until you become a client, we keep it only as long as we need it, and you can ask us to delete it at any time — write to communications@mindfullifechicago.com and we will.

3. Information collected automatically

Our website records limited technical information when you visit: your IP address, browser and device type, the pages you view, how long you spend on them, and the site or search that brought you here.

We use this to keep the site working, to understand which pages people find useful, and to defend against attacks. Our security software also logs login attempts and blocks suspicious traffic.

4. Cookies, analytics and advertising

We use Google Analytics to understand how the site is used — which pages are read, how people arrive, and where they leave. It sets cookies and collects the technical information described above.

We advertise on Google, and we measure whether those ads work. If you arrive from one of our ads and then contact us, Google records that the ad led to an inquiry. That is how we know which ads are worth paying for.

What we don’t do. We don’t build advertising audiences out of the pages you read here, and we don’t use the fact that you visited to follow you with advertising afterwards. We don’t run advertising pixels from social media platforms. And we don’t send Google, or anyone else, your name, your email address, or anything you type into our form.

Your choices. You can block or delete cookies in your browser settings, and blocking them won’t stop you using the site or contacting us.

5. Who we share information with

We do not sell your information, and we do not share it for anyone else’s advertising.

We use service providers that handle information on our behalf and only on our instructions:

  • Kinsta, which hosts the website
  • Google Workspace, which handles our email
  • Google, for the analytics described above

We may also disclose information where the law requires it, or where it’s necessary to protect someone from serious harm.

6. How we protect information

The site is served over an encrypted connection, administrative access is limited to the people who need it, we use security software that blocks suspicious traffic and repeated login attempts, and we keep the software current.

No website is completely secure. If a breach affects your information and the law requires us to notify you, we will.

7. Young people

We work with adolescents as clients, but this website is meant for adults. We don’t knowingly collect information through the website from anyone under 13. If you think a child has sent us information through the site, tell us and we’ll delete it.

8. Where we work

We provide therapy to people located in Illinois. The website can be reached from anywhere, and if you visit from a state or country with its own privacy laws you may have additional rights over the information we hold. Contact us and we’ll respond as that law requires.

9. Your choices

You can ask us to delete an inquiry you sent through the site, or ask what information we hold from your use of the website. Write to communications@mindfullifechicago.com.

Requests about your clinical record are handled under the Notice of Privacy Practices you received during onboarding, not under this policy.

10. Changes

If we change this policy we’ll update the effective date above, and note anything significant on this page.
